We welcome all inquires from our patients. We strive to offer our patients the most up to date care in a most efficient manner. We need your help to accomplish this goal! we will assist you in obtaining your records from your primary care doctors.

It is important to be prepared when you come to see the doctors:

What to Bring to Your 1st Appointment?

If you are a first-time patient, please plan on arriving at least 15 minutes earlier in order to complete a patient registration form if you have not printed the new patient information forms listed here on the right hand side. The new patient information form and new patient health forms are fillable. You may just download to your computer and complete them, save, and email back to us or print out to bring with you for your appointment.

Please bring the following items to the appointment:

• All Valid Insurance Card(s)
  
• One type of official ID (driver’s license, ID card, or passport)
• Referral forms (It is important for our HMO/POS patients to note that, according to the insurance companies that we participate with, all patients must obtain a referral from their primary care physician in order to see one of our cardiologists. You may be asked for your referral number when you call to make an appointment and you must have it at the time of your appointment)
  
• A complete LIST with doses of all prescription and Non-prescription medications which you are currently taking. 
  
• Any other records your doctor may have given you to bring to your appointment
• Any recent lab reports
• Any required co-payment on your insurance policy.
